Diagnosis
<p class="ql-align-justify">(1) Duodenal mucosa shows focal moderate villous atrophy associated i<span style="color: rgb(0, 29, 53);">ntraepithelial lymphocytosis and crypt hyperplasia </span>with submucosal mild infiltrate of lymphocytes and plasma cells. No evidence of specific granulomas. No evidence of atypia or malignancy. </p><p class="ql-align-justify">(2) Intact colonic mucosa with mild reduction in the number of mucigenic cells and mild infiltration by lymphocytes, plasma cells and neutrophils. However, crypt abscesses are not identified. No evidence of atypical or malignant cells.</p>
<p>Chronic diarrhea.</p>
<p class="ql-align-justify">Two biopsies were received :</p><p class="ql-align-justify">1- Duodenal biopsy : soft tissue fragments collectively measured 1 cm.</p><p>2- Colonic biopsy : soft tissue fragments collectively measured 0.6 cm.</p>
<h3><strong><em><u>(1) Duodenal endoscopic biopsy:</u></em></strong></h3><ul><li><strong>Focal moderate villous atrophy with associated chronic non-specific duodenitis, consistent with celiac disease. Modified Marsh-Oberhuber classification, grade B1.</strong></li><li><strong>Negative for malignancy. </strong></li></ul><h6><br></h6><h4><strong><u>COMMENT:</u></strong></h4><h5>Confirmatory correlation with clinical and laboratory chemical investigations is advised.</h5><p><br></p><h3><strong><em><u>(2) Colonoscopic biopsy:</u></em></strong></h3><ul><li><strong>Mild chronic colitis with minimal inflammatory activity. </strong></li><li><strong>Negative for epithelial dysplasia. </strong></li><li><strong>Negative for malignancy. </strong></li></ul>
