Sections show polypoid gastric mucosal fragments with hyperplastic gastric glands featuring prominent foveolar hyperplasia, characterized by pit elongation and tortuosity. Associated moderate inflammatory infiltrate is noted. Moderate activity is evidenced by neutrophilic infiltration of the glandular epithelium. Luminal bacteria consistent with Helicobacter pylori are present.
Entire gastric mucosa mildly hyperemic + multiple gastric fundal polyps, largest one 2 cm with ulcer on top, associated with atrophic antral gastritis.
Soft tissue fragments collectively measured 0.7 cm, totally embedded.
