Diagnosis
<ul><li>Myometrial tumors are benign moderately cellular leiomyoma formed of whorly bundles of mature smooth muscles separated by a fibrous stroma. No evidence of tumor necrosis. No evidence of atypia or malignancy. Polyp and endometrium show hyperplastic and non-secretory moderately branching glands separated by cellular stroma and lined by 1-2 layers of cells. Islands of these endometrial glands and stroma are seen entrapped superficial within myometrium. No evidence of complex glandular pattern. Cervix shows Nabothian cysts and mild endocervicitis. Lt ovary shows corpora albicans and cystic follicles. Both fallopian tubes are unremarkable.</li></ul>
<p>Ovarian mass.</p>
<p class="ql-align-justify">Total hysterectomy with bilateral salpingectomy and Lt oophorectomy specimen showing: uterus 9x7x4.5 cm with 2.2 cm thick wall showing two fibroid masses largest measure 3.5x3x3 cm. Endometrial thickness is 0.7 cm with polyp measured 1.3x0.7 cm. Cervix is 3 cm long. Lt ovary measured 2x1.5x1 cm, grossly unremarkable. Rt and Lt Fallopian tubes are 2.5 cm and 5 cm, respectively, both are grossly unremarkable.</p>
<p><strong><em><u>Total hysterectomy with bilateral salpingectomy and Left oopherectomy:</u></em></strong></p><ul><li><strong>Cellular leiomyomas. </strong></li><li><strong>Moderate simple endometrial hyperplasia.</strong></li><li><strong>Superficial adenomyosis.</strong></li><li><strong>Hyperplastic endometrial polyp. </strong></li><li><strong>Cervical Nabothian cyst with mild endocervicitis.</strong></li><li><strong>Left. ovarian cystic follicles and corpora albicans. </strong></li><li><strong>Fallobian tubes are unremarkable.</strong></li><li><strong>Negative for atypia and malignancy. </strong></li></ul>
