Both thyroid lobes show nodular adenomatous hyperplasia. Nodules are variable in size and composed of small and large crowded follicles full of thick colloid and lined by mature epithelium. Sections from some of the nodules show compact microfollicles devoid of colloid and lined by cuboidal cells separated by delicate vascular stroma. In these follicles nuclei are vesicular and look active but no appreciable mitosis. Stroma shows mild fibrosis. No evidence of hyperfunction or malignancy.
Two of the detected nodules in the separate fragments proved to be thyroid nodules with the same pathology described. One lymph node was detected showing reactive follicular lymphoid hyperplasia.

Total thyroidectomy specimen showing first lobe measuring 6.5x3x1.7 cm, second lobe measuring 8.5x5x2.8 cm and isthmus measured 2x1 cm. Thyroid capsule is intact. Serial sectioning revealed first lobe multiple nodules ranging in size from 0.6 cm to 1.3x1 cm, and multiple nodules in the Lt lobe ranging in size from 0.8 cm to 1.5 cm.
Received in the same container fibrofatty tissue fragment measured 1.7x1 cm entangling 3 nodules largest is 0.5 cm.
