Gastric mucosa showing focal superficial erosions and mild atrophy of glands with moderate infiltration by lymphocytes and plasma cells. Neutrophils are seen infiltrating glands thus indicating activity. H. pylori characteristic forms are seen colonizing glands. There is evidence of focal intestinal metaplasia but no evidence of atypia or malignancy
