(1) Sections show completely ulcerated urothelium with related malignant tumor formed of a dissociated population and fascicles of atypical spindle cells, oval, rounded and polygonal cells. Many of the cells have abundant peripheral dense eosinophilic cytoplasm. Nuclei are rounded, sometimes elongated, enlarged and vesicular with small nucleoli. Tumor sheets are incompletely separated by fibrotic or myxoid stroma. Tumor cells show marked anaplasia and active mitosis. Scattered multinucleated cells are also seen. Wide areas of necrosis are observed.
(2) All submitted fragments are complete necrotic and viable muscularis propria is not included in submitted material, hence proper assessment of stage of tumor is not possible. Possibilities of this tumor include spindle cell sarcoma (pleomorphic rhabdomyosarcoma and leiomyosarcoma) and sarcomatoid carcinoma. Marker study is recommended for definitive typing (Desmin, Myogenin, SMA, p63, GATA3 and CK).
