Submitted multiple fragments of bone infiltrated and partly replaced by a dense chronic inflammatory process composed of sheets and clusters of large foamy histiocytes with abundant vacuolated, eosinophilic cytoplasm, admixed with numerous lymphocytes, plasma cells, and scattered neutrophils. Interspersed are foci of necrotic and devitalized bony trabeculae surrounded by these inflammatory cells, with areas of granulation tissue and fibroblastic proliferation. Occasional multinucleated histiocytic giant cells are seen, but no well‑formed epithelioid granulomas, cholesterol clefts, or conspicuous atypical mitotic figures are identified. No malignant osteoid or overtly atypical spindle cell proliferation is seen in the examined sections. Overall features are those of a xanthogranulomatous chronic osteomyelitis involving bone
