Fragments of partially ulcerated esophageal and gastric mucosa with related infiltration by malignant large epithelial cells having irregular hyperchromatic nuclei arranged in irregular solid groups with occasional glandular differentiation. Anaplasia and mitosis are moderate to marked. Picture is that of poorly differentiated adenocarcinoma, grade 3. Correlation with endoscopic findings is recommended to identify the exact origin (stomach cardia or lower esophagus on top of Barrett's).
